The plan is available for FWD customers&#13;<br \/>\nwith individual medical, Voluntary Health Insurance Scheme (&#8216;VHIS&#8217;) or critical&#13;<br \/>\nillness insurance (&#8216;Medical and Critical Illness Insurance&#8217;), who are survivors&#13;<br \/>\nof cancer, heart attack or stroke. It also takes care of bereaved family&#13;<br \/>\nmembers, providing emotional support services in the unfortunate cases of customers&#13;<br \/>\npassing away (See Appendix 1 for details).<\/p>\n<p>\u00a0<\/p>\n<p style=\"text-align: center\">\u00a0<img src=\"https:\/\/images.media-outreach.com\/Thumb\/500x0\/196379\/FWD-Care-recovery-plan.jpg#image-196379\" width=\"500\"\/><\/p>\n<p align=\"center\">FWD&#8217;s revolutionary FWD Care recovery plan provides physical and&#13;<br \/>\nemotional support through a 6-month tailored programme to customers surviving from cancer, stroke, and heart attack, who would be&#13;<br \/>\nassisted by a registered Recovery Nurse throughout the whole recovery journey.<\/p>\n<p>\u00a0<\/p>\n<p>Demand for recovery support is soaring as medical advances increase&#13;<br \/>\ncritical illness survival rates. Studies show that cancer, stroke, and heart&#13;<br \/>\nattack are becoming more common among the 25 to 44 age group<a name=\"_ftnref1\" title=\"\">[1]<\/a>,<a name=\"_ftnref2\" title=\"\">[2]<\/a>,<a name=\"_ftnref3\" title=\"\">[3]<\/a> whose chances of survival are higher. However, typical recovery plans&#13;<br \/>\nin Hong Kong usually only provide treatment summaries and health care&#13;<br \/>\ninformation<a name=\"_ftnref4\" title=\"\">[4]<\/a>.&#13;<br \/>\nFWD Care aims to fill this service gap with a suite of professional and&#13;<br \/>\npersonalised psychosocial and physical rehabilitation services.<\/p>\n<p><b>Kelvin Yu, Chief Product Officer of FWD Hong Kong &amp; Macau<\/b>, said<b>, <\/b>&#8220;FWD Care takes a holistic approach in closing&#13;<br \/>\nthe recovery service gap with bespoke support for our customers and their&#13;<br \/>\nfamily members that meet their physical and emotional needs. By launching FWD&#13;<br \/>\nCare, we aspire to enhance the post-claim experience for our customers as we&#13;<br \/>\ncontinue realising our commitment to providing &#8216;better coverage, more&#13;<br \/>\ncomprehensive protection&#8217;.&#8221;<\/p>\n<p>FWD Care covers a wide range of professional services&#13;<br \/>\nand immediate care, ranging from consultation services provided by&#13;<br \/>\nphysiotherapists, sports therapists, Chinese medicine practitioners, dietitians,&#13;<br \/>\nand clinical psychologists to home assistants and transportation services. The assigned&#13;<br \/>\nRecovery Nurse will personalise a care plan and be at the customer&#8217;s side throughout&#13;<br \/>\nthe whole recovery journey once the claims are approved. This hassle-free&#13;<br \/>\nservice ensures that customers can focus on their recovery, overcome their&#13;<br \/>\nemotional challenges, and prepare for lifestyle readjustments.\u00a0<\/p>\n<p>FWD Care is also available for bereaved families of deceased&#13;<br \/>\ncustomers with an approved death benefit claim from FWD. It provides a family&#13;<br \/>\nmember, who is the spouse, a child, or a parent of the diseased customer, six months&#13;<br \/>\nof emotional support and home care services. During this time, a Recovery Nurse&#13;<br \/>\nwill help the family member cope with grief and assist in resuming a normal life.<\/p>\n<p><b><u>Appendix 1<\/u><\/b><\/p>\n<p>FWD Care recovery plan provides timely physical and psychological&#13;<br \/>\nsupport to the following customers in need:<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\nInsured persons of&#13;<br \/>\nFWD&#8217;s individual Medical and Critical Illness Insurance plans who have&#13;<br \/>\nsuccessfully claimed their crisis benefit or reimbursed their hospitalisation&#13;<br \/>\nand surgical expenses or hospital cash for cancer, heart attack or stroke,&#13;<br \/>\nwhich are the most common critical illnesses in Hong Kong; or<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\nAn immediate family&#13;<br \/>\nmember (i.e. spouse, child, or parent) of the deceased insured person with an&#13;<br \/>\napproved death benefit claim from FWD.<\/p>\n<p>\u00a0<\/p>\n<p>Service flow of FWD Care recovery plan:<\/p>\n<p>1.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\n<b>Reach out by FWD<\/b>: Once the eligible claim is approved from now until&#13;<br \/>\n30 June 2022 (both dates inclusive), FWD will contact the insured person or an&#13;<br \/>\nimmediate family member of the insured person.<\/p>\n<p>2.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\n<b>Provide consent<\/b>: The insured person or immediate family member will&#13;<br \/>\nreceive a request for consent to the terms of the service, including the&#13;<br \/>\nconsent for the transfer of personal data needed for FWD Care to provide&#13;<br \/>\nservices.<\/p>\n<p>3.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\n<b>Hear from a nurse<\/b>: An assigned Recovery Nurse will contact the participant&#13;<br \/>\nand be a companion throughout the recovery journey.<\/p>\n<p>4.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\n<b>Get the support you&#13;<br \/>\nneed<\/b>: The Recovery Nurse will discuss and coordinate the&#13;<br \/>\nsupport services best suited for the participant after he\/she has completed the&#13;<br \/>\nfirst consultation with a clinical psychologist or cardiologist. <\/p>\n<p>5.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#13;<br \/>\n<b>Plan completion<\/b>: The FWD Care recovery plan lasts for six months with&#13;<br \/>\nfull access to the Recovery Nurse during the period.<\/p>\n<p>\u00a0<\/p>\n<p>The FWD Care recovery plan&#13;<br \/>\nis provided by third-party service providers. FWD will not be responsible or&#13;<br \/>\nliable for any services offered by the third-party providers under the plan. Terms&#13;<br \/>\nand conditions apply to the plan.
