It is&#13;<br \/>\ncommon that people affected by this disease are not aware of its impact and how&#13;<br \/>\nto manage its symptoms effectively and confidently. In conjunction with World&#13;<br \/>\nParkinson&#8217;s Day this April, CERTAINTY would like us to take some time to&#13;<br \/>\nunderstand how this neurological disease can significantly impact the lives of&#13;<br \/>\nthe people it affects, especially when it comes to Activities of Daily Living&#13;<br \/>\n(ADL).<\/p>\n<p style=\"text-align: center\"><img src=\"https:\/\/release.media-outreach.com\/release.php\/Images\/Thumb\/500x0\/146819\/CERTAINTY-Product-visual.jpg#image-146819\" width=\"500\"\/><\/p>\n<p>&#13;<br \/>\n&#13;<br \/>\n&#13;<br \/>\n&#13; <\/p>\n<p>Today, there are more than 20,000 Malaysians diagnosed with&#13;<br \/>\nParkinson&#8217;s disease<b> <\/b>and this number&#13;<br \/>\nis only expected to rise as we move towards becoming an ageing nation.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Due to this decrease in dopamine, a Parkinson&#8217;s patient&#8217;s movements&#13;<br \/>\nare affected, resulting in common symptoms like tremors or trembling in the&#13;<br \/>\nhands, arms, legs, jaw or head; stiffness or rigidity in the limbs and body;&#13;<br \/>\nunusually slow movement; and impaired balance and coordination. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Those affected find it difficult to start or stop moving, and control&#13;<br \/>\nfine movements like gripping a pen and writing, as well as being prone to&#13;<br \/>\nfalling, making it challenging for them to carry out ADL. Their facial&#13;<br \/>\nexpressions can also become stiff, affecting their ability to communicate&#13;<br \/>\nnon-verbally. All these symptoms initially manifest subtly, e.g. the person&#13;<br \/>\nmight find it just a bit more difficult to get out of a chair or their bed, but&#13;<br \/>\nwill eventually progress if left untreated.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>In addition, those living with Parkinson&#8217;s disease\u00a0might&#13;<br \/>\nexperience symptoms like difficulty in chewing, swallowing and speaking; mood&#13;<br \/>\nchanges like depression and apathy; sleep problems; skin problems; cognitive&#13;<br \/>\nissues; constipation; and urinary problems like incontinence. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>All of these symptoms can have a profound impact on the patient&#8217;s&#13;<br \/>\nquality of life.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p><b>\u00a0<\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p><b>Managing&#13;<br \/>\nParkinson&#8217;s<\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Although the cause of Parkinson&#8217;s disease is still unknown and there&#13;<br \/>\nis no cure for the condition, there are a number of treatments that can help&#13;<br \/>\npatients control their symptoms for a better quality of life. These include&#13;<br \/>\nmedication, surgery and supportive therapy<sup><sup>[1]<\/sup><\/sup>.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>There are a variety of medicines available to treat Parkinson&#8217;s&#13;<br \/>\ndisease. Some are used during the early stages of the disease, while some are&#13;<br \/>\nprescribed for the more advanced stages. The drugs can be quite effective in&#13;<br \/>\ncontrolling the symptoms of the disease, but will eventually lose their effect&#13;<br \/>\nover time.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>The surgical option available for advanced Parkinson&#8217;s patients who do&#13;<br \/>\nnot respond well, or have stopped responding, to medication is deep brain&#13;<br \/>\nstimulation. This is where electrodes are placed in certain parts of the brain&#13;<br \/>\nand linked to a neurostimulator (also known as a brain pacemaker) in the chest,&#13;<br \/>\nwhich generates electrical impulses to help control the symptoms of the&#13;<br \/>\ndisease.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Supportive therapies like physiotherapy, occupational therapy and&#13;<br \/>\nspeech therapy, plus lifestyle modifications like exercise and dietary changes,&#13;<br \/>\ncan also help to ease the symptoms of this condition.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Physiotherapy can help a Parkinson&#8217;s patient maintain their&#13;<br \/>\nindependence by providing them with techniques to deal with movement issues,&#13;<br \/>\ne.g. when getting in and out of bed, a chair or a car, as well as strength and&#13;<br \/>\nbalance training to help with movement difficulties and preventing falls. A&#13;<br \/>\nphysiotherapist will also be able to tailor an exercise and fitness programme&#13;<br \/>\nfor the patient.<sup><sup>[2]<\/sup><\/sup><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Meanwhile, occupational therapy provides practical advice and methods&#13;<br \/>\nfor patients to carry out their ADL, while coping with symptoms like tremors,&#13;<br \/>\nanxiety, fatigue and difficulty in communicating. Among the ways an&#13;<br \/>\noccupational therapist can help those with Parkinson&#8217;s are demonstrating&#13;<br \/>\ndifferent methods of accomplishing daily tasks, like buttoning up a shirt or&#13;<br \/>\nputting on make-up; recommending practical changes to the house or furniture&#13;<br \/>\narrangement to take into account their symptoms; and helping the patient&#13;<br \/>\ndevelop strategies to cope with ADL that might become more difficult as their&#13;<br \/>\ndisease progresses in the future.<sup><sup>[3]<\/sup><\/sup><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>For patients in the early stages of Parkinson&#8217;s, a speech therapist&#13;<br \/>\nwill help them maintain their ability to communicate for as long as possible.&#13;<br \/>\nIn the more advanced stages of the disease, the therapist will be able to&#13;<br \/>\nsuggest techniques and tools to help ease communication. In addition, speech&#13;<br \/>\ntherapy also helps the patient with eating problems like difficulty chewing and&#13;<br \/>\nswallowing, or drooling. This form of therapy also provides tips to caregivers&#13;<br \/>\nand family members of the patient on how best to communicate with the patient,&#13;<br \/>\nas well as warning signs of when the patient may be experiencing eating&#13;<br \/>\ndifficulties.<sup><sup>[4]<\/sup><\/sup><\/p>\n<p>&#13;<br \/>\n&#13;<br \/>\n&#13;<br \/>\n&#13; <\/p>\n<p>While exercise is an important part of a healthy lifestyle, it is of&#13;<br \/>\nparticular importance to those with Parkinson&#8217;s disease, as it helps them&#13;<br \/>\nmaintain their balance, mobility and ability to perform ADL.<sup><sup>[5]<\/sup><\/sup>&#13;<br \/>\nThe earlier a Parkinson&#8217;s patient starts a regular exercise routine, the longer&#13;<br \/>\nthey will be able to maintain a better quality of life and delay progression of&#13;<br \/>\ntheir symptoms. While there is no one &#8220;right&#8221; workout regime for Parkinson&#8217;s&#13;<br \/>\npatients, each person should ensure that they cover the fitness elements of&#13;<br \/>\nstrength training, aerobic activity, flexibility, agility and balance for the&#13;<br \/>\nbest results. Tai chi and yoga in particular have been shown to help with&#13;<br \/>\nbalance, flexibility and mobility in patients with Parkinson&#8217;s.<sup><sup>[6]<\/sup><\/sup><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>When it comes to diet, there is no one food or combination of foods&#13;<br \/>\nthat have been proven to help with Parkinson&#8217;s disease. However, simple&#13;<br \/>\nmodifications to what a patient eats can help to alleviate some of the symptoms&#13;<br \/>\nthey are experiencing. For example, constipation can be ameliorated by&#13;<br \/>\nincreasing the amount of fibre in one&#8217;s diet and by drinking lots of water,&#13;<br \/>\nwhile low blood pressure and its accompanying symptoms like dizziness upon&#13;<br \/>\nstanding up, can be managed by increasing the amount of salt in one&#8217;s diet and&#13;<br \/>\neating small, regular meals. For patients with difficulty eating, their foods&#13;<br \/>\nmight have to be softer in texture, while also nutrient-rich to ensure that&#13;<br \/>\nthey are getting sufficient nutrition.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>There are also various aids available that can help to make life a&#13;<br \/>\nlittle easier for Parkinson&#8217;s patients. For example, walking sticks, walking&#13;<br \/>\nframes, grab rails and handrails can help with moving around outside or inside&#13;<br \/>\nthe house, including while using the toilet or shower. Satin or silk sheets or&#13;<br \/>\npyjamas can help patients slide onto and out of bed more easily. There are also&#13;<br \/>\nall kinds of modified eating and kitchen utensils, like spoons, forks, cups,&#13;<br \/>\nplates, bowls and non-slip table mats, that can help make eating easier for a&#13;<br \/>\nParkinson&#8217;s patient. Other simple modifications include putting rubber bands&#13;<br \/>\naround items to provide a better grip and using magnets or Velcro instead of&#13;<br \/>\nbuttons on clothes.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p><b>Urinary&#13;<br \/>\nincontinence<\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>One other symptom of Parkinson&#8217;s disease that can have a major impact&#13;<br \/>\non quality of life is urinary incontinence, where the patient is unable to&#13;<br \/>\nfully control the passing out of their urine. This is due to the disruption in&#13;<br \/>\ncommunication between the brain and the bladder due to low dopamine levels. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>The types of urinary incontinence Parkinson&#8217;s patients might&#13;<br \/>\nexperience are urge incontinence and nocturia.<sup><sup>[7]<\/sup><\/sup> Urge&#13;<br \/>\nincontinence occurs when a person gets that urgent need to urinate, but is&#13;<br \/>\nunable to control themselves from urinating before they get to the toilet.&#13;<br \/>\nMeanwhile, nocturia is the abnormally frequent need to get up and pass urine&#13;<br \/>\nduring the night while you are asleep.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>As can be imagined, urinary incontinence can have a negative impact on&#13;<br \/>\na person&#8217;s mental and social health. It is not unusual for such persons to&#13;<br \/>\nexperience feelings of shame, embarrassment, social isolation, loneliness and&#13;<br \/>\nloss of control, as well as body image issues. The lack of ability to control&#13;<br \/>\ntheir urination will cause them to limit their social activities outside the&#13;<br \/>\nhouse for fear of wetting themselves unintentionally, resulting in social&#13;<br \/>\nisolation and loneliness. They also tend to reduce or stop working out as&#13;<br \/>\nhigh-impact exercises can lead to involuntary leakage of urine. These issues&#13;<br \/>\nare further compounded when the person is also struggling with other symptoms&#13;<br \/>\nof Parkinson&#8217;s disease, and can lead to mental health issues like depression&#13;<br \/>\nand anxiety.\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Urinary incontinence can also be difficult to deal with for&#13;<br \/>\ncaregivers. The financial burden of continence care, the relationship changes&#13;<br \/>\nthat happen and the social isolation enforced upon them can affect the&#13;<br \/>\ncaregiver negatively.<sup><sup>[8]<\/sup><\/sup><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>Fortunately, urinary incontinence is a manageable condition.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>For a start, the patient can &#8220;train&#8221; their bladder by establishing a&#13;<br \/>\nregular routine of going to the toilet, e.g. after every meal. They should&#13;<br \/>\navoid drinking any beverages starting two hours before bedtime, as well as&#13;<br \/>\nminimise their intake of caffeine, gassy drinks and alcohol as these can&#13;<br \/>\nstimulate urine production. Wearing clothes that have less buttons or zips, and&#13;<br \/>\ncan be easily removed quickly, can also help Parkinson&#8217;s patients who struggle&#13;<br \/>\nwith both difficulty in movement and urinary incontinence.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p>In addition, there are various products available for those with&#13;<br \/>\nurinary incontinence to help manage their condition.
