All recommendations by the Task Force&#13;<br \/>\nhave since been implemented.<a name=\"_ftnref2\" title=\"\">[2]<\/a><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">It is&#13;<br \/>\nthe responsibility of all parties involved to be proactive and work&#13;<br \/>\ncollaboratively to ensure the continued accessibility of quality healthcare in&#13;<br \/>\nSingapore. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\"><b><u>Greater&#13;<br \/>\ntransparency required from all parties <\/u><\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">We&#13;<br \/>\nagree that greater transparency will provide more clarity on this matter. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">On this&#13;<br \/>\nend, we recently concluded an update of the LIA Singapore guidelines on panel management.&#13;<br \/>\nThis includes guidance that IP insurers should explain the criteria for&#13;<br \/>\nselection of panel doctors and is publicly available on the LIA Singapore&#13;<br \/>\nwebsite<a name=\"_ftnref3\" title=\"\">[3]<\/a>.&#13;<br \/>\n\u00a0<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">Equally,&#13;<br \/>\nwe urge SMA to respond to our repeated calls for greater transparency too. LIA&#13;<br \/>\nSingapore has requested for clinical quality measures and guidelines to prevent&#13;<br \/>\nover-treatment on numerous occasions. We ask that SMA respond promptly and&#13;<br \/>\npositively on this. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">Clinical&#13;<br \/>\nquality measures will enable insurers to make better, evidence-based decisions&#13;<br \/>\non admitting doctors into panels. Policyholders will ultimately benefit because&#13;<br \/>\nthey will receive better care.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\"><b><u>Expanding&#13;<br \/>\npanels to include all private specialists may result in the need for further&#13;<br \/>\npremium increases <\/u><\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">IP&#13;<br \/>\npanels have been implemented for several years now and are not completely new&#13;<br \/>\nto the IP market. Panels have expanded over time, and LIA Singapore&#8217;s guidance&#13;<br \/>\nto IP insurers is that panels should be sufficient to offer a wide range of&#13;<br \/>\nmedical services to policyholders. IP insurers have an interest in ensuring&#13;<br \/>\nthat their panels are comprehensive as this increases panel usage, which helps&#13;<br \/>\ninsurers better manage costs.<\/p>\n<p class=\"xmsonormal\">SMA is&#13;<br \/>\nstrongly advocating for panels to be expanded to include all private&#13;<br \/>\nspecialists in Singapore, and for all panel doctors to be compensated up to the&#13;<br \/>\nupper bounds of the MOH fee benchmark range. This needs to be carefully&#13;<br \/>\nconsidered as tradeoffs are involved.<\/p>\n<p class=\"xmsonormal\">The average&#13;<br \/>\nratio of upper bound to lower bound for surgeon fee benchmarks is 1.8<a name=\"_ftnref4\" title=\"\"><sup><sup>[4]<\/sup><\/sup><\/a>. As&#13;<br \/>\nsuch, proceeding per SMA&#8217;s suggestion without calibration may lead to cost&#13;<br \/>\nincreases and further premium increases for policyholders.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">Striking&#13;<br \/>\nthis balance is critical, and we invite SMA for a discussion on how this can be&#13;<br \/>\ndone in a way that will not disproportionately burden policyholders with higher&#13;<br \/>\npremiums. <\/p>\n<p>&#13;<br \/>\n&#13;<br \/>\n&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\"><b><u>Misleading&#13;<br \/>\nanalysis should be avoided<\/u><\/b><\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">It is&#13;<br \/>\nimportant that organisations put out objective analyses which avoid biased&#13;<br \/>\nconclusions. <\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">In this&#13;<br \/>\nregard, we find SMA&#8217;s analysis of insurers&#8217; costs and claims costs to be&#13;<br \/>\nmisleading. Claim increases are the main driver of premium increases. LIA&#13;<br \/>\nSingapore will fully address this in due course, along with more detailed&#13;<br \/>\ncomments on the rest of SMA&#8217;s position statement.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p><b><u>Importance of collaboration to achieve our shared goal of affordable,&#13;<br \/>\nquality healthcare for Singaporeans <\/u><\/b><\/p>\n<p>&#13;<br \/>\n&#13;<br \/>\n&#13;<br \/>\n&#13; <\/p>\n<p>Our&#13;<br \/>\ncollective priority must be to focus on finding the best ways to achieve a&#13;<br \/>\ncommon goal for the benefit of Singaporeans.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">LIA&#13;<br \/>\nSingapore has always pursued an open, discussive and constructive dialogue to&#13;<br \/>\nreceive feedback and resolve potential issues. Updating of the abovementioned&#13;<br \/>\nLIA guidelines on panel doctor and, previously, the standard LIA&#13;<br \/>\nPre-Authorisation Form<a name=\"_ftnref5\" title=\"\">[5]<\/a>&#13;<br \/>\nbased on SMA&#8217;s inputs were also actioned on swiftly.<\/p>\n<p>&#13;<br \/>\n&#13; <\/p>\n<p class=\"xmsonormal\">Accordingly,&#13;<br \/>\nwe look forward to greater collaboration amongst all parties with the formation&#13;<br \/>\nof a trilateral committee, comprising LIA Singapore, SMA and the Academy of&#13;<br \/>\nMedicine, Singapore already set up by the Ministry of Health (MOH) to discuss&#13;<br \/>\nand resolve IP-related issues, and to refine the implementation of HITF&#13;<br \/>\nrecommendations together in a constructive manner. <\/p>\n<p>&#13;<br \/>\n&#13;<br \/>\n&#13;<br \/>\n&#13;
